Functional Capacity Assessment
The purpose of a functional capacity assessment is to gather and organize comprehensive information about you and your needs. The functional capacity assessment will identify your strengths, current support systems, areas for improvement, and goals. Functional capacity assessments are a valuable tool to initiate the goal-setting process, develop therapy and support plans, and periodically review and update these plans to stay informed and any changes in an individual's circumstances. A Functional Capacity Assessment typically involves:
1. A home visit.
We will interview you. Your family/caregivers can also provide input.
2. The next step is to observe you doing a range of activities

Home visit: Evaluation of self-care tasks, domestic tasks, organisational skills.

Community: Assessment of skills such as money management, social skills, road safety, and public transport.

With this information, we may ask you to complete some standardised assessments. The standardised assessments gauges an individual's current abilities and the level of assistance you require to complete each task.
3. Finally we will create a concise written report that summarises the interview, observations, and the standardised assessment results. We will compile the information, highlighting strengths and providing recommendations. We will discuss the report with you to ensure it meets your needs. ​
